LIABLITY RELEASE AND MEDIA/MEDICAL CONSENT FORM
Panoramic Voices is excited that your child(ren) will be participating in the Panoramic Voices Youth Choir! In order for your children to participate in the Panoramic Voices (“PV”) Youth Choir (“Choir”), PV requires the legal guardians of all participants to agree to certain actions and to provide certain releases and consents. As such, PV must receive this signed Liability Release and Media/Medical Consent Form (“Form”) for every participant prior to the start of each Choir project.

LIABILITY RELEASE. Legal Guardian acknowledges and understands that engaging in Choir activities may involve certain risks, including accidents, injuries, and exposure to airborne communicable illnesses, and that it is not possible to foresee and protect the Children from all possible dangers. Legal Guardian agrees to communicate to the Children all safety and behavior expectations so that the Children understand what is expected of them. Participation in the Choir is voluntary, and Legal Guardian assumes all risk relating to the Children’s participation in Choir activities. Without limiting the foregoing, Legal Guardian also acknowledges and understands that PV uses contracted/donated space for its activities (including rehearsals and concerts) and PV is not legally responsible for ensuring the safety or accessibility of such spaces. Legal Guardian hereby releases PV and all of PV’s directors, officers, employees, staff, contractors, volunteers, agents, and representatives, (all of which shall be collectively referred to as “PV Parties”) from any and all claims, causes of action, injuries, damage, or expenses relating to or arising out of Choir programming, events or other activities (“Claims”), whether or not such Claims arise out of the acts or omissions of PV Parties or other participants in the Choir, or the removal of the Children from Choir activities due to behavior, safety, or other issues.
MEDIA CONSENT. PV will create, collect, and/or use materials relating to the Choir for its event announcement, social media, marketing, and other purposes. This may include materials that include the names, photos, videos, voice recordings, spoken or written words, or other similar materials of the Children (“Materials”). These materials may be used on social media (such as Facebook, Instagram, and other platforms), on TV, on the radio, in newspapers or magazines, in email, and/or other media that can be viewed by the general public. By signing this Form, Legal Guardian consents to the creation, collection, and/or use of Materials, which may include material about the Children, for any Choir-related purpose. In addition, Legal Guardian consents to PV’s sole ownership of all the Materials (including, without limitation, all intellectual property rights relating to such Materials), and to PV’s use and/or modification of the Materials in its sole discretion. Legal Guardian understands and agrees that Legal Guardian will not be paid for the use of the Materials, and Legal Guardian will not ask to be paid for the use of the Materials. Without limiting the general release stated above, Legal Guardian hereby releases PV Parties from any claims whatsoever relating to the Materials, even if the Materials contain typographical errors, distortions, or other alterations.
MEDICAL CONSENT. Legal Guardian understands and agrees that it is Legal Guardian’s affirmative responsibility to provide PV with written information about the Children’s allergies or other medical conditions. PV will use reasonable efforts to contact the individuals named on the Children’s emergency contact form in case of a medical emergency. Legal Guardian hereby consents to PV providing care to the Children as may be determined to be reasonable under the circumstances, which may include administering of medication (e.g., Epipens), calling for emergency medical care, or other action(s). Legal Guardian understands and agrees that the cost of all treatment administered to the Children is solely the responsibility of the Legal Guardian. If the Children have any known medical or health issues that may need adult supervision, Legal Guardian agrees to have an adult accompany the Children to all Choir activities. Without limiting the general release stated above, Legal Guardian hereby releases PV Parties from any claims whatsoever relating to any medical needs or emergencies relating to the Children.
Legal Guardian acknowledges and agrees that Legal Guardian has had an opportunity to ask questions about this Form and that Legal Guardian acknowledges, understands, and agrees to its contents.
